Grace Community Care & Homes seeks a part-time PMHNP in New Jersey to conduct psychiatric evaluations, prescribe medications and coordinate with the medical director and clinical team. Start date and duties depend on program approvals and payer credentialing.

Remote responsibilities include assessing patients, developing treatment plans, prescribing within NJ authority, monitoring responses, managing refills, and telehealth case reviews. On-site work or IOP/residential duties may be disclosed.

Qualificações

• Active NJ RN/APN credentials and psychiatric certification.
• Required prescribing credentials and practice arrangements.
• Experience with the target population.
• Experience with psychiatric evaluations, medication management and telehealth.

Responsabilidades

• Perform psychiatric evaluations and medication management.
• Prescribe medications within New Jersey authority.
• Coordinate with medical director and clinical team.
• Develop treatment plans and monitor responses; manage refills.
• Maintain electronic records and participate in virtual case reviews.
