Job summary

Pathways to Life seeks a Community Supports Team Substance Abuse Professional to support individuals with co-occurring substance use and mental health conditions. You will perform assessments and person-centered treatment planning in coordination with CST partners to tailor services to each member.

You will monitor symptoms, provide substance use education, promote positive leisure activities, and participate in crisis planning to ensure continuous care within the continuum.

Qualifications

• Must hold one of the NC substance use credentials: LCAS, LCAS-A, CADC or CCS.
• Experience in mental health or substance use services is preferred.
• Ability to collaborate with CST partners and treat clients with co-occurring disorders.

Responsibilities

• Perform assessments and person-centered treatment planning in coordination with CST partners.
• Assist in symptom monitoring and management; provide substance abuse education.
• Promote positive leisure activity development and skill-building to support recovery.
• Assist in crisis planning and renewing authorization to ensure continued services.
