Job summary

Pathways to Life, Inc. seeks a Community Supports Professional to support individuals with co-occurring substance use and mental health needs in Roxboro, NC. The role emphasizes assessments, treatment planning, and coordination with the CST to promote recovery and wellness.

You will work with a multidisciplinary team to monitor symptoms, deliver substance use education, and foster community engagement while empowering clients through person-centered supports.

Qualifications

• Must hold North Carolina substance use credentials.
• Experience in substance abuse treatment and co-occurring disorders.
• Ability to collaborate with CST partners and community resources.

Responsibilities

• Perform assessments and person-centered treatment planning with CST partners.
• Assist in symptom monitoring and education to reduce higher levels of care.
• Promote positive leisure activity development and skill building for recovery.
• Assist in crisis planning and renewing authorization to ensure continuum of care.
