Job summary

Pathways to Life, Inc. is seeking a Substance Abuse Specialist to join our Community Supports Team. You will walk alongside members managing substance use and co-occurring mental health challenges, helping them build skills and connections for lasting recovery and improved quality of life.

You will coordinate care under the Team Lead, participate in assessments, crisis planning, and recovery-oriented activities, and engage with community resources to expand our service area.

Qualifications

• Required Credential — one of the following North Carolina substance use certifications.
• Physical Requirements: ability to walk/stand, lift up to 25 lbs, valid driver's license, CPR certification.

Responsibilities

• Conduct assessments and develop person-centered treatment plans in coordination with CST partners.
• Monitor symptoms and deliver substance use education to reduce higher levels of care.
• Assess and develop crisis plans tailored to each member's needs.
• Promote leisure activity development and skills to support sustained recovery.
• Help members build tools and habits for long-term wellness.
• Assist in authorizations so members receive uninterrupted care.
• Collaborate with the treatment team while managing your caseload.
• Build relationships with behavioral health resources across the community.

CCS (Certified Clinical Supervisor)
