The role requires experience with underserved populations in clinical settings; licensure such as LMSW/LCSW or LPC/LADAC is preferred.

Qualifications

• Experience providing counseling to underserved populations in a clinical setting.
• Lived addiction experience is a plus.
• Licensed or eligible for LMSW/LCSW or LCPC/LPCLADAC credentials.

Responsibilities

• Provides individual counseling to highly complex patients.
• Conducts clinical assessments to determine treatment needs.
• Designs and updates treatment plans with care coordination and discharge planning.
• Manages caseload and complies with clinical treatment requirements.
• Collaborates with nurses and providers to deliver holistic care.
• Facilitates urine drug screenings.
