Crossroads Treatment Centers is seeking a Substance Abuse Counselor in Knoxville, TN. The role focuses on individual counseling and clinical assessments within an outpatient MAT framework, supporting patients through treatment planning and care coordination.

The position emphasizes collaboration with the clinical team to deliver holistic services, manage caseloads, and ensure adherence to treatment requirements.

Qualifications

• General experience working with underserved populations in a clinical setting.
• Lived addiction experience is a plus.

Responsibilities

• Provides individual counseling to highly complex patients.
• Conducts clinical assessments to appropriately assess patient's treatment needs.
• Designs and updates the patient’s treatment plan, including care coordination, referrals, and discharge planning.
• Manages assigned caseload and compliance with clinical treatment requirements.
• Collaborates with nurses and providers to provide holistic treatment services.
• Facilitates urine drug screening for patients.
