Job summary
Crossroads Treatment Centers in Chattanooga, TN seeks a Substance Abuse Counselor to provide individual counseling to highly complex patients, perform clinical assessments, and design/update treatment plans with care coordination and discharge planning. The role manages a caseload and works with nurses to deliver holistic treatment services, including urine drug screening.
Qualified candidates will hold LMSW/LCSW/LAPC/LPC/LADAC licenses; lived addiction experience is a plus.

- Lived addiction experience is a plus.
- Qualified candidates will hold LMSW, LCSW, LAPC, LPC, or LADAC licenses.
Responsibilities
- Provides individual counseling to highly complex patients.
- Conducts clinical assessments to determine treatment needs.
- Designs and updates the patient’s treatment plan, including care coordination, referrals, and discharge planning.
- Manages assigned caseload and ensures compliance with clinical requirements.
- Collaborates with nurses and providers to deliver holistic treatment services.
- Facilitates urine drug screening for patients.
