Job summary

The Vermont State Colleges System seeks a Health & Wellness Case Manager to support students within the Student Affairs team. You will manage a caseload of students with complex challenges, coordinating interventions and connecting them with campus and community resources to promote academic success and well-being.

The role involves crisis response, assisting with medical and mental health leave processes, and guiding students back to campus with re-entry planning in collaboration with CARE

Qualifications

• Bachelor’s degree in counseling or related field and at least 1 year of experience in case management, counseling, residence life, advising, or related field.
• Ability to manage sensitive situations with professionalism, discretion, and sound judgment.

Responsibilities

• Manage a caseload of students with complex concerns (mental health, medical, academic, personal).
• Conduct individualized assessments and develop tailored intervention and support plans.
• Provide ongoing case management with regular follow-up, monitoring, and documentation.
• Facilitate referrals to campus and community resources for coordinated support.
• Serve as a resource to CARE teams and participate in threat assessment processes.
• Support students in crisis, including mental health emergencies and significant disruptions.
• Provide guidance to faculty, staff, and families regarding students of concern.
• Assist students with medical leave and return-to-campus processes, including re-entry planning.
• Provide follow-up support for reintegration after leave.
• Develop targeted interventions and programming for at-risk student populations.
• Collaborate with campus partners to support student success and retention.
• Build relationships with off-campus providers and resources.
• Develop and deliver training for faculty and staff on identifying and supporting students of concern.
• Promote student wellness through education and communication.
• Maintain accurate case records in line with FERPA, HIPAA, and institutional policies.
• Analyze trends to inform intervention strategies and resource allocation.
• Contribute to assessment of program effectiveness and outcomes.
• Maintain a limited clinical caseload to support crisis care.
