Job summary

The Vermont State Colleges System invites applications for a Health & Wellness Case Manager to join the Student Affairs team. You will manage a caseload of students with complex concerns, deliver individualized interventions, and coordinate referrals to campus and community resources to support retention and wellbeing.

Qualifications

• Bachelor's degree in counseling, social work, higher education, public health, or related field plus one year of student-support experience.
• Demonstrated ability to manage sensitive situations with professionalism, discretion and sound judgment.
• Master's degree and licensure preferred.

Responsibilities

• Manage a caseload of students with complex concerns and deliver tailored intervention plans.
• Conduct individualized assessments, provide ongoing case management and documentation.
• Coordinate referrals to campus and community resources for timely support.
• Support crisis response and assist with medical leave processes and return-to-campus planning.
• Collaborate with CARE teams and campus partners to ensure coordinated student-centered care.
• Promote student wellness through proactive programming and outreach.
• Maintain confidential case records in compliance with FERPA, HIPAA, and institutional policies.
