Job summary

The Health and Wellness Case Manager is a key member of the Student Affairs team, providing coordinated support to students with complex challenges. You will manage a caseload, deliver individualized interventions, and connect students with campus and community resources to support retention and well-being.

You will collaborate with CARE teams, assist with crisis response, medical leave processes, and reintegration planning while maintaining confidentiality and FERPA/HIPAA compliance.

Qualifications

• Bachelor’s degree in counseling, social work, higher education, public health plus 1 year of relevant experience.
• Master’s degree preferred and licensure in field.
• Ability to manage sensitive situations with professionalism and sound judgment.

Responsibilities

• Manage a caseload of students with mental health, medical, academic, and personal challenges.
• Conduct individualized assessments and develop tailored intervention plans.
• Provide ongoing case management with follow-up and documentation.
• Facilitate referrals to campus and community resources.
• Support students in crisis and during medical leave processes.
• Collaborate with CARE teams and campus partners to ensure coordinated support.
• Promote student wellness through proactive programming and training for staff.
