Job summary

The Vermont State Colleges System seeks a Health & Wellness Case Manager to support students facing complex challenges and maintain their academic success. You will manage a caseload, deliver individualized interventions, and connect students with campus and community resources, contributing to retention and timely transitions back to the University.

You will collaborate with CARE teams and campus partners, provide crisis support, assist with medical and mental health leaves, and document case

Qualifications

• Bachelor's degree in counseling, social work, higher education, public health, or a related field is required.
• Experience supporting student populations in case management, counseling, residence life, or advising.
• Strong ability to maintain confidentiality and exercise professional judgment.

Responsibilities

• Manage a caseload of students with complex concerns across mental health, medical, academic, and personal challenges.
• Conduct individualized assessments and develop tailored intervention plans.
• Provide ongoing case management with follow-up, monitoring, and documentation.
• Facilitate referrals to campus and community resources for coordinated support.
• Collaborate with CARE teams and campus partners to ensure student-centered responses.
• Assist students with medical leave processes and return-to-campus planning.
