Job summary

Socket.dev is seeking a Health & Wellness Case Manager to join the Student Affairs team. You will manage a caseload of students with complex challenges, deliver individualized interventions, and connect students to campus and community resources to support retention and success.

You will coordinate crisis response, assist with medical and mental health leave processes, and guide students back to campus. You will collaborate with CARE teams, document plans, and help faculty and families navigate

Qualifications

• Bachelor's degree plus one year of experience supporting student populations in case management, counseling, residence life, advising, or related field.
• Demonstrated ability to manage sensitive situations with professionalism, discretion, and sound judgment.

Responsibilities

• Manage a caseload of students experiencing elevated or complex concerns, including mental health, medical, academic, and personal challenges.
• Conduct individualized assessments and develop tailored interventions and support plans.
• Provide ongoing case management, including regular follow-up, monitoring, and documentation.
• Facilitate referrals to campus and community resources, ensuring coordinated and timely support for students.
• Serve as a resource to CARE teams and participate in threat assessment team processes, providing case insight and recommendations.
