Job summary
University of Kentucky's Center for Support and Intervention is seeking a Case Manager to provide case management services to students needing additional resource support to improve well-being, safety and academic success. The role involves assessment of needs, linkage to on- and off-campus resources, and thorough documentation for an assigned caseload.
You will work with UK faculty, health care providers and staff, coordinate referrals, and contribute to outreach and education efforts.
Qualifications
- Experience providing case management services in higher education or community settings.
- Knowledge of student mental health and campus resources.
- Ability to collaborate with faculty, healthcare providers, and staff.
- Strong interpersonal, communication, and organizational skills.
- Proficiency with MS Windows, MS Office (Word, PowerPoint) and database management.
Responsibilities
- Provide case management for a caseload of students.
- Assess student needs and connect to campus and community resources.
- Create cases, conduct outreach and document services.
- Coordinate interventions, referrals and follow-up.
- Collaborate with UK staff and campus partners.
