Job summary

The University of Kentucky, Center for Support and Intervention, is seeking a Case Manager to provide comprehensive case management to students in need of additional resource support to improve well-being, safety and academic success.

The role reports to the Assistant Director and involves needs assessment, resource navigation, case documentation, and outreach for an assigned caseload, collaborating with faculty, healthcare providers, and campus partners to support students throughout their

Qualifications

• Experience providing case management services, either in a higher education or community based setting.
• Knowledge of issues facing student populations with lower retention rates and knowledge of campus and community resources.
• Excellent interpersonal, decision making and verbal and written communication skills; strong attention to detail and planning and organizational skills.

Responsibilities

• Provide direct, caring support to students through needs assessment and connection to resources on and off-campus.
• Manage case creation, outreach to and engagement of students, and all case documentation tasks for an assigned caseload.
• Facilitate interventions, referrals and follow-up services based on identified student needs.
• Collaborate with UK Faculty, health care providers, Office for Student Success staff, parents and other UK staff, and participate in divisional committees and initiatives.
