Job summary

University of Rochester seeks a CARE Case Manager to provide strengths-based assessment, intervention, and coordination of services for students facing academic, personal, or medical challenges. The role supports referrals, collaborates with campus partners, and advances holistic well-being across the university community.

The position may require occasional evening and weekend hours and involves working with the Office of the Dean of Students and other campus departments to implement programs

Responsibilities

• Maintains a caseload of CARE referrals for students in need of additional support.
• Conducts initial review of CARE referrals using the NABITA rubric.
• Receives referrals from the Behavioral Intervention Team (BIT) to provide case management services and deliver interventions.
• Schedules and meets with students, assesses safety and needs, coordinates action plans, referrals, and follow-up.
• Communicates with referrers and provides updates, collaborating with staff, offices, and resources.
• Closely manages high risk cases including concerns such as suicidal ideation and/or attempt, substance use, serious mental health issues.
• Documents notes in the Advocate electronic record-keeping system.
• Monitors and reassesses utilization, completion, and efficacy of interventions; amends action plans as needed.
• Co-facilitates CARE Team and Student Support Network meetings under the Director’s leadership.
• Coordinates with campus and community resources.
• Manages CARE Network in the Director’s absence and provides leadership on student support.
• Creates trainings, workshops, and presentations pertaining to student support and well-being.
• Represents CARE at Resource Fairs, Orientation, Accepted Students Days, and other events.
• Outreach to faculty and staff to recognize and support students in need.
• Develops marketing outreach initiatives that improve understanding and usage of CARE.
