Job summary

Southeast Community College is seeking a Faculty Student Care Case Manager to provide holistic, student-centered case management for students facing complex personal and academic challenges. You will assess needs, develop intervention plans, and coordinate with campus partners to promote student well-being.

The role requires strong communication, confidentiality, and documentation skills, with duties including crisis support, progress evaluation, and collaboration with Dean of Students and CAPS

Qualifications

• Broad knowledge of student development, mental health care, and appropriate services/supports.
• Knowledge of post-secondary education, admissions, registration, and financial aid processing/procedures.
• Understanding of higher education case management, FERPA regulations.

Responsibilities

• Assess individual student needs and develop intervention plans.
• Coordinate multidisciplinary responses and monitor progress.
• Maintain accurate documentation of CARE Team activities and outcomes.
• Support campus crises and provide student support after incidents.
