Job summary

Northwestern Medicine's BCVI Clinical Nurse Coordinator role supports expanding structural heart programs (TAVR, TEER) and EP, primarily on site at CDH in the West Region. Travel to Delnor and Kish is limited for PTO coverage only.

You will coordinate care, manage referrals, and lead clinical conferences with the cardiovascular team. Under supervision, you’ll organize patient care plans, educate patients and families, and ensure data-driven workflows.

Qualifications

• Bachelor’s degree in nursing is required.
• Minimum 2–5 years experience in cardiovascular surgery and/or cardiology.
• Current IL nursing license and BLS certification.

Responsibilities

• Plan, coordinate, direct, and provide clinical care to designated patients.
• Coordinate ongoing care with interdisciplinary team to maximize wellness.
• Develop education for patients and families and serve as education resource.
• Act as communication link between patient, family, caregivers, and team.
• Coordinate follow-up visits, referrals, testing, and long-term care as needed.
• Schedule clinical conferences and prepare meeting content and presentations.
• Utilize data to identify patients and ensure appropriate care plans.
• Contribute to quality initiatives and ensure compliance with policies.
