Job summary

Northwestern Medicine is seeking a BCVI Clinical Nurse Coordinator to support the hypertension program across the system. You will coordinate care for cardiovascular patients, act as a liaison with referring physicians, and organize clinical conferences. This role requires a BSN, valid IL license, BLS, and 2–5 years in cardiovascular care.

Qualifications

• Bachelor’s degree in nursing required.
• Current IL nursing license required.
• Current Basic Life Support certification required.
• Minimum 2–5 years experience in Cardiovascular surgery and/or Cardiology.
• Experience in hospital or clinic setting with direct physician interaction.

Responsibilities

• Plan, coordinate, direct, and provide appropriate clinical care to patients in the designated setting.
• Collaborate with interdisciplinary team to coordinate ongoing care and optimize wellness.
• Develop and provide education for patients, families, and staff; serve as resource for ongoing education.
• Act as a communication link between patient, family, caregivers, and interdisciplinary team.
• Coordinate care including follow-up visits, referrals, testing, and long-term follow-up.
• Schedule and organize clinical conferences; prepare materials and presentations.
• Utilize data to identify patients and determine appropriate care pathways.
• Contribute to quality improvement initiatives and policy adherence.
