Job summary

Northwestern Medicine is seeking a BCVI Clinical Nurse Coordinator to support growing structural heart programs (TAVR, TEER) and EP, primarily on-site at CDH in the West Region. Travel to Delnor and Kish is limited and only for urgent coverage.

The RN coordinates patient care, referrals, and conferences, organizing clinical workflows and educating patients and families. Under supervision of Medical Directors, this role drives cardiovascular care planning and team collaboration.

Qualifications

Bachelor’s degree in nursing required.
Current IL nursing license and BLS certification.
Minimum 2–5 years in cardiovascular surgery or cardiology.
Experience in hospital/clinic settings with physician interaction.
MSN and specialty certification preferred.

Responsibilities

Planning, coordinating, directing, and providing appropriate clinical care to patients in the designated care setting.
Works with interdisciplinary team to coordinate ongoing care and cost-effective pathways.
Develops and provides education for patients, families, and staff.
Serves as a communication link between patient, family, caregivers and team.
Coordinates follow-up visits, referrals, testing, and long-term care.
Schedules and organizes clinical conferences; prepares content and materials.
Utilizes data to identify care pathways and improve workflows.
Contributes to quality initiatives and ensures policy compliance.
