Job summary

Saint Mary’s Health Network, a member of Prime Healthcare, seeks a Stroke Coordinator to lead coordination of the Stroke Program in collaboration with the Performance Improvement manager/director. The role includes chart reviews, data abstraction for CMS core measures, and staff education on quality indicators.

Ideal candidates have a healthcare degree, RN licensure or related certification, and strong MS Office skills, with the ability to work with a multidisciplinary team on process

Qualifications

• Bachelor’s degree in healthcare field preferred.
• State RN licensure or healthcare field certification preferred.
• Excellent computer skills including MS Word, Excel, Access.

Responsibilities

• Coordinate the Stroke Program with the Performance Improvement team.
• Conduct chart reviews and data abstraction for CMS Core Measures.
• Educate staff on core measures and updates to indicators.
• Collaborate with multidisciplinary teams to identify problems and implement improvements.
