Job summary

Saint Mary’s Health Network, a member of Prime Healthcare, seeks a Stroke Coordinator to lead coordination of the Stroke Program in collaboration with the Performance Improvement manager/director. The role includes chart reviews, data abstraction for CMS core measures, and staff education on quality indicators.

Responsibilities

In collaboration with the manager/director of Performance Improvement, the Stroke coordinator is responsible for coordinating the Stroke Program and assisting in the coordination, integration, and implementation of the organization wide performance improvement activities to ensure compliance with company policies and state/federal regulatory and accreditation standards. Activities include but are not limited to chart reviews, data abstraction related to stroke performance and CMS Core Measures and other quality outcomes studies, and staff education regarding core measures and updates regarding new and enhance indicators. Collaborates with multidisciplinary team on problem identification and resolution, cost containment issues, implementation of new services and system/performance measures for both Risk and Stroke programs.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

• 1. Bachelor’s degree in healthcare related field preferred.
• 2. State RN licensure or Certification in a healthcare field preferred.
• 3. Excellent computer skills are required including proficiency in (MS Office (Word, Excel, Access).
• 4. Experience in reviewing charts for care issues.
• 5. Detail oriented organizational skills.
• 6. Must be able to handle multiple cases, directions and be able to follow-through.
• 7. Good communication skills both verbal and written.
• 8. Experience with Medical Staff communication a must.
• 9. Excellent interpersonal relationship skills with exceptional professional work ethics.
• 10. Proficient in word processing, spreadsheets, database and typing.
