The University of Maryland Medical System is looking for a Stroke Program Coordinator in Largo, Maryland. This role involves serving as a clinical expert liaison for stroke patients, coordinating care, and ensuring adherence to best practices.
Candidates must possess a current Maryland RN license and have at least 2 years of experience as a stroke coordinator. Compensation ranges from $44.76 to $67.19 per hour, along with other benefits and shift differentials.
Reporting to the Director of Clinical Programs, the Stroke Program Coordinator serves as a clinical expert liaison between physicians, nursing, and other UMCAP departments regarding the care of acute stroke patients.
Participates in daily rounds for stroke patients and coordinates patient care during and after hospitalization. Serves as a liaison between physicians and other members of the healthcare team to develop comprehensive care plans for cardiovascular patients.
Maintains clinical knowledge and skills for the stroke patient population and completes ongoing staff educational opportunities for all levels and departments interacting with the cardiac population.
Participates in planning, development, and presentation of special projects. Provides leadership in developing, revising, and implementing best practices related to stroke care, including rapid diagnosis and management.
Effectively communicates with all hospital disciplines, patients, families, and the public to achieve excellence in inpatient care.
Creates and ensures all policies are current and reviewed annually.
Facilitates screening community events, coordinates screening stations, arranges for guest speakers, and conducts monthly committee meetings to review quality measures and implement improvement plans.
Collects and analyzes ongoing data regarding outcomes of the Stroke Program and submits required data to the appropriate regulatory agencies.
Provides a level of care and interventions consistent with the highest quality and appropriate length of stay; facilitates discharge and follow‑up care. Demonstrates the ability to prioritize and provide direction when workloads change during crisis situations.
Documents in patient charts as needed. Meets daily with the unit charge nurse and case manager to solve unresolved patient issues, facilitate admissions, transfers, and discharges; increase compliance with Core Measures and MIEMMS standards, avoid insurance denial, and reduce delays in service. Reviews physician’s orders to ensure that PT and OT evaluations have occurred, and that short‑term rehab assessments, blood sugar management, laboratory tests, and x‑rays are complete and appropriate.
Collaborates with other members of the healthcare team in planning, implementing, monitoring, and evaluating comprehensive care for cardiac patients and their families.
Assesses the learning needs of patients, families, and staff in collaboration with all members of the healthcare team, plans, provides, and coordinates patient/family and staff education for those involved in the care of the neurologic patient.
Assists with the accreditation process to ensure designation of Stroke Center is maintained.
Participates in performance improvement initiatives and quality monitoring.
Collaborates with the Nursing PI Coordinator, Program Medical Director, and the VP of Nursing to identify areas of improvement within the Stroke Programs.
Coordinates with other hospital and/or health system services in the continuum of care when care cannot be provided within this facility. Does related work as assigned.
