Job summary

AdventHealth in Palm Coast, FL is seeking a Stroke Program Coordinator to lead educational planning, oversee accreditation readiness, and drive improvements in stroke care across the care continuum.

The role requires RN licensure with 2+ years in stroke/critical care and progressive leadership experience. Responsibilities include committee leadership, policy development, and collaboration with campus leadership to maintain accreditation.

Job Description:

Coordinates the development and deployment of stroke educational plans, communicating updates on outcomes, process changes, and regulatory requirements. Organizes community education events to raise stroke awareness and prevention strategies. Develops and reviews evidence-based clinical pathways to standardize stroke care, ensuring timely implementation and updates. Evaluates stroke program metric performance, ensuring accurate reporting and sharing outcomes with committees and leadership. Rounds on stroke patients to provide education and support, participates in multidisciplinary rounds, and responds to stroke alert events. Conducts periodic follow-up calls to determine patient outcomes and provide additional education and support. Other duties as assigned. Serves as the stroke program clinical content expert, utilizing knowledge of clinical stroke evidence-based guidelines, national accreditation standards, and regulatory requirements. Leads the stroke interdisciplinary committee(s) with the stroke medical director to improve clinical performance, standardize processes, and ensure compliance with standards. Identifies variations in care and develops corrective action plans to improve performance. Leads mortality and morbidity committee reviews to identify opportunities for improving clinical outcomes. Oversees the initial and ongoing national accreditation process, including readiness rounds, mock stroke alerts, education, and documentation maintenance. Acts as the stroke program lead during accreditation site visits, collaborating with campus leadership to maintain accreditation.

  • Knowledge related to telemedicine practice [Preferred]
  • Knowledge of American Heart Association/American Stroke Association Get with the Guidelines Stroke Registry [Preferred]
  • Knowledge of state/federal regulatory agencies, accreditation standards and national society clinical guidelines [Preferred]
  • Quality improvement knowledge include but not limited to Centers for Medicare & Medicaid Services (CMS) outcomes, Value Based Purchasing, Bundle Payment, and other national quality improvement initiatives [Preferred]

  • Associate's of Nursing [Required]

Work Experience:
  • 2+ critical care, neuroscience, or stroke care related nursing experience [Preferred]

  • 5+ of acute care, critical care, neuroscience, or stroke related nursing experience [Required]
  • Experience as stroke coordinator or with accreditation of stroke programs [Preferred]
  • Experience working in large healthcare systems neuroscience service line [Preferred]
