Job summary

Boston Medical Center is seeking a Comprehensive Stroke Center RN Navigator to lead stroke education and care coordination within the Neuro Stroke Center. You will collaborate with bedside nurses, therapists, case managers, social workers, and the Neurology team to drive patient education and outcomes.

The role requires an RN with a BSN and 5+ years of stroke experience, strong EPIC skills, and exemplary communication.

Qualifications

• RN licensure in Massachusetts required.
• BLS certification required.
• ACLS preferred; Stroke Certification preferred.
• Minimum 5 years of RN experience in stroke/cerebrovascular care.
• Experience working with diverse, underserved populations.

Responsibilities

• Lead stroke education initiatives with the care team.
• Participate in quality assurance and performance improvement projects.
• Maintain and improve data capture for clinical care and outcomes.
• Audit charts and support adherence to Joint Commission and other standards.
• Coordinate with multiple departments to ensure coordinated patient care.
