Responsibilities

• Provide post-discharge care coordination through 7-day and 90-day follow-up calls and mRS assessments.
• Educate patients and families on stroke diagnosis, treatment, prevention, and medication management.
• Assess adherence to secondary prevention therapies and monitor for side effects and mental health concerns.
• Schedule outpatient testing, referrals, and follow-up appointments.
• Advocate for patients and connect with community stroke resources and support groups.
• Identify social determinants of health and coordinate referrals and support services.
• Collaborate with care team to escalate unresolved concerns and ensure timely physician follow-up.
• Participate in community outreach, stroke awareness, health fairs, screenings, and education.
