Qualifications

• Three years nursing in acute care setting.
• Bachelor Degree and RN licensure required.
• Experience coordinating care for stroke patients preferred.

Responsibilities

• Provide post-discharge care coordination through 7‑day and 90‑day follow‑ups.
• Educate patients and families on diagnosis, treatment options, and secondary prevention.
• Assess adherence to therapies and monitor for medication side effects.
• Schedule outpatient testing, referrals, and follow‑ups.
• Advocate for patients and connect them with resources and support services.
• Identify barriers to care and coordinate appropriate referrals.
• Collaborate with interdisciplinary care team for timely follow‑up.
• Participate in community outreach and stroke awareness activities.
