Job summary

Family & Children’s Agency (FCA) in Norwalk, CT, seeks a street outreach case manager to provide immediate access to coordinated entry, basic needs, and other services for those experiencing unsheltered homelessness. The role focuses 70% of time on canvassing in the community from 9 Mott Ave, Norwalk.

Essential qualifications include a BA in a related field, bilingual Spanish, and experience with homeless populations and mental health/substance use disorders.

Qualifications

• BA in related field preferred.
• Bilingual/Spanish Speaking a must.
• Experience working with homeless population and mental health/substance use disorders.
• Reliable transportation and a valid Driver’s License.
• Awareness of cultural and socioeconomic characteristics of clients served.

Responsibilities

• Conduct street outreach to locations frequented by unsheltered individuals.
• Engage clients with sensitivity to their needs and distrust of traditional services.
• Connect with people in community settings and inform them about housing and services.
• Provide crisis counseling and immediate support.
• Offer practical assistance (meals, blankets, clothing, toiletries).
• Publicize services within CAN/CoC and coordinate entry appointments.
• Assist navigation of homeless service system with CAN Navigators.
• Provide information and referrals to housing, healthcare, mental health, substance abuse, employment, and education programs.
