Job summary

Columbus House is hiring a Full Time (32hours/week) Outreach Case Manager in Middletown, CT. The role focuses on engaging adults experiencing homelessness, linking them to social services, and supporting stabilization and sobriety.

Under the Middlesex Emergency Services Manager, you will conduct outreach, coordinate with partner agencies, and maintain thorough documentation in the HMIS database. Strong English communication and crisis-intervention skills are required.

Qualifications

• Minimum of two years’ experience in human services or related fields.
• Excellent organizational and time management skills with a deadline-oriented mindset.
• Strong written and oral communication skills in English.
• Proficiency in Microsoft Office and other common desktop applications.
• Valid drivers’ license with a good driving history.

Responsibilities

• Provide linkages for clients to social, mental health, educational, vocational, and substance abuse services.
• Provide transportation to appointments for clients.
• Coordinate and collaborate with other agencies and social service providers.
• Engage in street outreach in all kinds of weather conditions.
• Attend agency, program and network meetings weekly and monthly.
• Monitor cases and complete documentation/charting for Columbus House and funders.
• Enter and maintain client charts accurately in HMIS.
• Support a 17-client case load.
