Job summary

Newport Mental Health in Middletown, RI is seeking a Case Manager–Street Outreach to engage clients in shelters and local housing authorities, developing pathways to housing with partner support.

The role requires 2 years of street-based outreach with an associate degree (bachelor's preferred). Excellent communication, empathy, and self-motivation are essential as you coordinate with the team and community providers.

Qualifications

• 2 years minimum experience providing street-based services to highly vulnerable populations.
• Excellent verbal and writing skills with the ability to communicate with diverse audiences.
• Caring and empathetic, with the desire to help others.

Responsibilities

• Provide outreach, navigation, and supportive services to individuals experiencing homelessness due to mental health, substance use, or co-occurring disorders.
• Support clients in accessing resources in the community and build relationships with staff and providers.
• Adhere to housing-first and harm-reduction models while maintaining respectful interactions.
• Listen to client needs and coordinate with supervisor for supervision.
• Develop ties to community, law enforcement, and other homeless service providers.
• Link clients to bridge/interim housing programs and help enroll in permanent housing-related programs.
