St Johns Community Health inLos Angeles seeks a Certified Peer Recovery Navigator – Street Medicine to support unhoused individuals and others with barriers to care. You will provide outreach, harm reduction education, SBIRT-related activities, and recovery coaching within a multidisciplinary Street Medicine team.

You will help clients identify goals, access medical, behavioral health, housing and benefits services, and coordinate referrals while documenting activities in the EHR.

Qualificações

• Medi-Cal Peer Support Services Specialist certification is required OR completion of a state-approved peer support training program and successful certification required.
• Must maintain certification and meet applicable California requirements for delivery of Medi-Cal Peer Support Services.
• High School diploma or GED; Associate degree preferred.
• Experience working with people experiencing homelessness is strongly preferred.
• Bilingual Spanish/English preferred.

Responsabilidades

• Street-Based outreach and engage participants in low-barrier settings.
• Provide harm reduction education and peer recovery support.
• Coordinate care navigation and linkage to MAT and behavioral health services.
• Document services and outcomes in the electronic health record.
• Collaborate with multidisciplinary Street Medicine team and partners.
