Healthcare in Action, Inc. seeks a Peer Navigator to support unhoused individuals in Hemet, CA. You will build rapport, motivate behavior changes, and connect members with housing, medical, and social services through the Street Medicine model.

You will document encounters, coordinate with the Navigation Center staff, and accompany patients to appointments, often traveling to meet needs in the community.

Qualifications

• G.E.D./High School required plus 3+ years of work experience.
• Associate's Degree plus 2 years of work experience.
• Bachelor’s degree preferred plus 1+ years of work experience.
• Direct work experience with patients experiencing homelessness is valued.
• Ability to work in outdoor environments and travel 50%+.

Responsabilités

• Build rapport with members to motivate positive behaviors.
• Coach members to implement strategies for health improvement.
• Connect members to programs addressing care barriers.
• Link members to local services and coordinate care needs.
• Document encounters and update care plans in systems.
• Schedule encounters based on acuity and work with supervisors.
