Job summary

Healthcare in Action in Hemet, CA seeks a Peer Navigator to assist people experiencing homelessness in managing their health. You will engage with participants, provide education, and connect them to housing resources, care, and medications through our street medicine model.

Under supervision, you will schedule encounters, document in the EMR, advocate for clients, and promote self-management for better health outcomes while coordinating with local services.

Qualifications

• G.E.D./High School, required plus 3+ years of work experience.
• Associate's Degree plus 2 years of work experience.
• Bachelor's degree preferred plus 1+ years of work experience.
• Experience with patients experiencing homelessness or related outreach.

Responsibilities

• Build rapport with members to encourage positive behavior changes.
• Coach members to implement strategies and connect to programs.
• Link members to local services and follow up on care needs.
• Provide accompaniment to medical/dental appointments and schedule encounters.
• Collaborate with Navigation Center staff and HIA team on care management.
• Document interactions in the EMR and care plans in HMIS/Coordinated Entry.
• Work under supervision of a Social Worker and uphold patient-facing duties.
