Job summary

Healthcare in Action is seeking a Peer Navigator for the San Fernando Valley to support unhoused individuals through education, housing and resource navigation, and care coordination in ECM and CSS programs. The role emphasizes personal experience sharing, motivation for health behavior change, and collaboration with a street-medicine team.

The position requires direct field work, the ability to work independently and as part of a team, and reliable transportation.

Qualifications

• GED/High School required plus 3+ years of relevant work experience.
• Associate's Degree with 2+ years of relevant experience.
• Bachelor’s degree preferred with 1+ year of relevant experience.
• Experience with homelessness, mental health or substance use populations is valued.

Responsibilities

• Build rapport with members to encourage positive health behaviors.
• Coach members to plan and implement strategies to reduce health risks.
• Connect members to local, county and state services and resources.
• Follow up with members and advocate for their care needs.
• Travel up to 50% and accompany members to appointments when needed.
• Document encounters and update HMIS/Coordinated Entry System records.
