Job summary

Healthcare in Action seeks a Peer Navigator for Riverside, CA to support unhoused patients through street medicine, education and peer-led care coordination. You will relay experiences to motivate health improvements, connect clients with housing and community resources, and assist with transportation to appointments.

Role emphasizes teamwork, independent work, and handling multiple tasks outdoors while maintaining accurate documentation in EHR/HMIS systems.

Qualifications

• GED/High School required plus 3+ years of work experience in related fields.
• Associate's degree preferred plus 2 years of work experience.
• Bachelor’s degree preferred plus 1-2 years of work experience.

Responsibilities

• Build rapport with members by communicating personal experiences to motivate behavior change.
• Coach members to implement strategies to minimize health risks.
• Connect members to local, county, and state services/resources.
• Escort members to medical and social services appointments as needed.
• Document care management activities in the EHR and HMIS.
