Job summary

Healthcare in Action is seeking a Peer Navigator to support people experiencing homelessness in the South County Riverside area, delivering care through street medicine and coordinating with Medicare/Medicaid plans and health systems.

You will build rapport, connect members to housing and health services, document encounters in the electronic health records system, and accompany patients to appointments, with travel up to 50% and supervision by a Social Worker.

Qualifications

• G.E.D./High School, required plus 3+ years of work experience.
• Associate's Degree plus 2 years of work experience.
• Bachelor's degree preferred plus1+ years of work experience.

Responsibilities

• Build rapport with members to motivate behavior changes.
• Coach members to implement strategies for health improvements and connect to programs.
• Link members to local, county and state services and follow up.
• Provide accompaniment and support to medical/dental appointments.
• Collaborate with Navigation Center staff for care management.
