Job summary

Healthcare in Action is a San Diego-based organization delivering street-medicine services to people experiencing homelessness, contracting with Medicare/Medicaid plans and hospitals. The Float Peer Navigator role provides continuity of care, panel coverage, and field support across Hemet and South County Riverside areas, including onboarding and Medi-Cal activities.

It is a full-time, Monday–Friday, 8–5 position.

Qualifications

• G.E.D./High School required plus 3+ years of work experience.
• Associate's degree preferred plus 2 years of work experience.
• Bachelor's degree preferred plus 1-2 years of work experience.
• Work experience with patients experiencing homelessness, including those with mental health or substance use conditions.
• Ability to travel 50% or more and a valid driver's license.
• Proficiency with computer systems such as Epic and HMIS.

Responsibilities

• Build rapport with members using trauma-informed engagement and peer support to encourage positive behavior change.
• Coach members to create strategies addressing health risks and barriers to care.
• Connect members to medical, behavioral health, housing, benefits and community services; follow up as advocate.
• Accompany members to appointments when support is needed outside street medicine.
• Promote self-management, self-efficacy, and appointment adherence.
• Maintain documentation across Epic, HMIS, and housing platforms.
• Schedule encounters based on acuity and program requirements using scheduling tools.
• Collaborate with care teams to ensure escalation and care coordination.
• Maintain assigned member panel and meet productivity and compliance expectations.
• Travel between Hemet and South County Riverside service areas as directed.
