Job summary

Healthcare in Action is seeking a Peer Navigator to support unhoused patients in Anaheim, CA. The role involves motivating behavioral changes, connecting to resources, and providing care coordination in a street-medicine setting.

You will work with the Navigation Center and field teams to document encounters, manage plan of care, and accompany patients to appointments, while promoting self-management and resilience.

Qualifications

• Excellent communication and interpersonal skills.
• Ability to work in outdoor environments and coordinate care.
• Proficiency with computers and multiple systems (timekeeping, call center software).
• Willingness to travel 50% or more with valid driver’s license.

Responsibilities

• Build rapport with members to encourage positive behavior changes.
• Connect members to programs to address barriers to care.
• Provide accompaniment and support for medical appointments and care coordination.
• Enter encounter notes and care plans into the EHR system.
• Schedule encounters based on member acuity and coordinate with team.
