Job summary

Rockwell Park Rehabilitation and Healthcare Center in Charlotte, NC is hiring a dedicated MDS Coordinator to join our care team. You will coordinate MDS processes, develop care plans with the MDS, and monitor plans for residents while ensuring regulatory compliance and accurate documentation.

The role requires an active state nursing license, MDS experience, CPR certification, and strong knowledge of MDS processes.

Qualifications

• MDS experience required.
• Current State Nurse License required.
• CPR certification required.
• Knowledge and training in all aspects of the MDS process.

Responsibilities

• Keep abreast of current federal and state regulations and standards.
• Assist with development of comprehensive care plans with MDS coordination.
• Coordinate and monitor the written plan of care for each Resident.
• Assist nursing management with coordination, management and review of nurse’s notes.
• Monitor Resident status changes and ensure timely team involvement.
• Ensure MDS documentation meets regulatory and auditor requirements.
• Perform regular audits of documentation for accuracy.
• Assist with discharge processes and administrative reporting.
