Job summary

UVM Health - UVM Medical Center seeks a Health Access & Affordability Project Coordinator to support statewide efforts across Vermont and Northern New York, reducing financial, transportation, digital, and other nonclinical barriers to care.

Under the Community Health Improvement Manager, the Coordinator organizes project activities, facilitates collaboration among partner hospitals, clinical and operational teams, and community organizations, maintains timelines and deliverables, and tracks

Qualifications

Bachelor's degree in public health, community health, social work, human services, health care administration, public administration, or a related field required.
Two to four years of relevant experience in project coordination, community health, public health, or health care access preferred.

Responsibilities

Coordinate project activities and timelines across multiple departments and partners.
Facilitate collaboration among UVM Health partner hospitals, clinical and operational teams, and community organizations.
Gather input from patients and community members to identify access challenges and inform improvements.
Track progress and outcomes; support Health Equity and Community Health priorities.
