Job summary

Lamoille Health Partners in Morrisville, Vermont, seeks a Clinical Care Coordinator to join our Community Health Team. You will guide patients through health, insurance, and social systems to achieve their goals, in partnership with primary care and behavioral health staff.

This role emphasizes compassionate communication, care coordination, and helping underserved rural populations access appropriate services while maintaining patient privacy and quality care.

Qualifications

  • Active professional license or certification aligned with Vermont standards (e.g., mental health counseling, social work, or addictions).
  • Compassionate communicator with ability to build trust and practice culturally sensitive care.
  • Expert understanding of family systems, community resources, and insurance complexities.
  • Team player in an interdisciplinary environment with proficiency in EMR and patient privacy.

Responsibilities

  • Holistic support and counseling to assess social, emotional, and financial factors and help patients set health goals.
  • Resource navigation to connect patients with community resources, including sliding fee scales and post-hospital discharge care.
  • Brief interventions and counseling using evidence-based tools to support patients navigating distress.
  • Care coordination as the bridge between individuals, families, and multidisciplinary providers for seamless communication.
  • Contribute to quality improvement projects and PCMH initiatives to enhance patient care standards.

Holistic Support & Counseling: Use motivational interviewing and a whole-person approach to assess social, emotional, and financial factors, helping patients build actionable health goals.

Resource Navigation: Connect patients with essential community resources, including sliding fee scales, low-cost medications, transportation support, and post-hospital discharge care.

Brief Interventions: Provide brief individual counseling and utilize evidence-based tools (like CALM or CAMS models) to support patients navigating acute distress or barriers to external clinical care.

Care Coordination: Serve as the bridge between individuals, families, and multidisciplinary providers to ensure seamless communication and continuity of care.

Quality & Growth: Participate in patient-centered medical home (PCMH) initiatives, team huddles, and quality improvement projects (like PDSA cycles) to continuously enhance our patient care standards.

Licensed & Qualified: You hold an active professional license or certification aligned with Vermont standards (e.g., mental health counseling, social work, or addictions) and follow best practices outlined by your respective governing board.
