Job summary

WMCH Weirton Medical Center is seeking a Clinical Relations Advocate to drive growth in referrals and admissions across the assigned territory. The role partners with hospitals, SNFs, and primary care practices to educate teams on hospice and home health services, eligibility basics, and referral pathways.

You will engage physicians, nurses, social workers, and discharge planners in person or virtually to support timely screenings.

Qualifications

• Must possess one of the following licenses issued by the state board: RN, PT, or OT.
• Two (2) years clinical experience in home health, hospice, palliative care, care transitions, rehabilitation, or hospital-based case management.
• Bachelor of Science in a related healthcare discipline preferred; CHPN/CCM certification preferred.

Responsibilities

• Strengthen and grow referral volume and admissions across the assigned territory by building purposeful relationships with hospitals, SNFs, PCPs, specialty groups, palliative care teams, and community organizations.
• Provide education to physicians, nurses, social workers, discharge planners, and care coordinators on hospice and home health services, eligibility basics, benefits, and referral pathways.
• Meet with patients and families to explain services, support decision-making, and connect to the clinical intake team for timely screening.
• Coordinate with intake team and leadership to ensure timely communication and admissions.
• Document outreach activity and referral outcomes in CRM/EMR systems and analyze trends.
• Plan and deliver educational programs for clinical and community audiences.
• Review performance metrics with leadership and refine outreach strategies for territory growth.
• Promote compliant, high-quality admissions and escalate concerns when needed.
• Participate in team meetings, QAPI activities, community events, and agency initiatives.
