Job summary

WMCH Weirton Medical Center is seeking a clinical liaison to strengthen referrals and admissions for hospice and home health services. You will work with hospitals, SNFs, and primary care to expand partnerships and ensure smooth patient transitions.

Role requires RN/PT/OT licensure as applicable, with at least two years in home health or related fields; five years in hospice/home health preferred. Strong communication and data-driven outreach skills are essential.

Qualifications

• Current RN license issued by the state or multi-state license via eNLC.
• Licensed Physical Therapist license in the service state or applicable compact.
• Licensed Occupational Therapist license in the service state or OT compact.
• Two years of clinical home health, hospice, palliative care, care transitions, rehab, or case management.
• Bachelor's in a related healthcare discipline; CHPN/ CCM certifications preferred.
• Five years of hospice or home health experience preferred.

Responsibilities

• Grow referral volume and admissions by building relationships with hospitals, SNFs, primary care, and community groups.
• Educate physicians, nurses, social workers, discharge planners, and care coordinators about hospice/home health services.
• Meet patients and families to explain services, answer questions, aid decision-making, and connect to intake.
• Respond to inquiries from referral partners and coordinate with intake and leadership for timely communication.
• Document outreach, patient interactions, partner communications, and referral outcomes in CRM/EMR; track trends.
• Plan and deliver educational programs (in-services, lunch-and-learns, presentations) for clinical/community audiences.
• Review performance metrics with leadership; use data to identify opportunities and guide territory growth.
• Ensure compliant, high-quality admissions by sharing information and escalating concerns to licensed clinicians when needed.
• Participate in team meetings, continuing education, QAPI activities, and agency initiatives.
• Represent the organization with professionalism, compassion, integrity, and service excellence; drive regional growth.
• Use data analytics to inform decisions, adjust territory plans, and support business development goals.
