Job summary

WMCH Weirton Medical Center seeks an experienced Clinical Relations Advocate to strengthen referrals, educate partners, and guide patients through home health and hospice decisions. You will collaborate with clinical leaders and intake to ensure timely admissions and exceptional patient experiences across the service area.

The role emphasizes relationship-building, data-driven outreach, and adherence to quality standards while advancing regional growth and care transitions.

Qualifications

• Must possess one of the following licenses: RN, PT, or OT.
• Two years clinical experience in home health, hospice, palliative care, care transitions, rehabilitation, or hospital-based case management.
• Five years of experience in hospice or home health (preferred).

Responsibilities

• Strengthen and grow referral volume and admissions across the territory by building strategic relationships with hospitals, SNFs, primary care practices, specialty groups, palliative care teams, and community organizations.
• Educate referral partners on hospice and home health services, eligibility, benefits, and referral pathways.
• Meet with patients and families to explain services, answer questions, support decision-making, and connect to intake team for screening.
• Coordinate with intake team and leadership to ensure timely communication and accurate referrals.
• Document and track outreach activity, patient interactions, partner communications, and referral outcomes using CRM or EMR systems.
• Plan and deliver educational programs (in-services, lunch-and-learns, presentations) for clinical and community audiences.
• Review performance metrics with leadership and use data to refine outreach strategies and growth plans.
• Promote compliant admissions by elevating clinical or regulatory concerns to licensed clinicians when needed.
• Represent the organization with professionalism and compassion in all interactions.
