Job summary

Qualifications

• Bachelor’s degree in a mental health discipline is required.
• Certification of case management training is required.
• Knowledge areas include psychiatric and substance use disorders in adults, cultural diversity, crisis intervention, rehabilitation principles, community resources, adult education methods, and benefits entitlements (SSA/Medicaid/Medicare).

Responsibilities

• Coordinate and deliver Stepping Up program services with the Alabama StateStepping Up Coordinator, TA Team, and local partners.
• Conduct case management assessments and develop individualized plans; implement plans with jail/hospital staff; link clients to community services.
• Advocate for clients with providers; monitor progress; ensure quality services and participate in Stepping Up trainings and alliance meetings.
• Complete needs assessments and develop service plans; conduct home visits and assist clients in securing services.
• Maintain accurate clinical records per AltaPointe standards and SUN-R assessments; document progress and dispositions.
• Manage caseload to meet productivity standards and ensure respectful, confidential client interactions.
• Participate in performance improvement activities and mandatory trainings.
