Job summary

Qualifications

• Bachelor’s degree in behavioral health or related discipline preferred.
• Must meet all driving requirements.
• Knowledge of psychiatric disorders in adults.
• Knowledge of crisis intervention and non-violent crisis procedures.

Responsibilities

• Assesses consumer needs and documents goals and plans in ISP.
• Provides linkage to community resources and monitors progress.
• Travels to homes, schools, and community settings to support consumers.
• Collaborates with treatment teams and advocates for least restrictive placements.
• Documents in SUN-R framework and maintains clear progress notes.
• Participates in Performance Improvement activities and safety protocols.
