Montgomery Area Mental Health Authority, Inc. is seeking an Adult Case Manager for the Montgomery County Stepping Up Program.

The role focuses on intensive case management for adults with serious mental illness and co-occurring disorders, coordinating with jails, hospitals, and community partners to reduce recidivism and improve access to services. The position requires a Bachelor's in Behavioral Science with two years of related experience, a valid driver's license, and Medicaid provider

Qualifications

• Bachelor's degree in Behavioral Science or related field and two years direct, related experience.
• Must meet requirements for Approved Medicaid Provider Status.
• Must hold a valid Driver's License and maintain a driving record acceptable to insurance carrier.

Responsibilities

• Provide intensive case management services to screen, assess, develop a case plan and link consumers to appropriate mental health and social services.
• Coordinate with Montgomery County stakeholders (jails and emergency rooms) to screen for mental illness and refer to Stepping Up program.
• Participate in local planning committee to support policies and community outreach.
• Complete SUN Assessment Form on each client.
• Develop an Individual Service Plan based on needs and update as required.
• Implement plan with county jails and area hospital staff.
• Assist in obtaining social day services, housing, transportation, and income support.
