Carastar Health in Montgomery, AL seeks an Adult Case Manager to support the Stepping Up program, providing intensive case management for adults with serious mental illness and co-occurring disorders.

The role coordinates with state partners, jails, and hospitals, develops individual service plans, links clients to services, and ensures proper documentation and reporting within the electronic health record. Evening and weekend hours may be required; valid driver's license preferred.

• Knowledge of psychotropic drugs and their effects.
• Ability to assess mental status and determine needs.
• Strong interpersonal and collaboration skills with clients and staff.
• Willing to work evenings/weekends when required.
• Reliable transportation for client transport.

Mansioni

• Provide intensive case management, screen, assess, develop a plan and link to services.
• Coordinate with local jails and ERs to screen for mental illness and refer to Stepping Up.
• Participate in planning committee to support local policies and outreach.
• Complete SUN Assessment Form for each client.
• Develop and update Individual Service Plans.
• Implement plan with county jails and hospital staff.
• Assist in obtaining housing, daily living, and income support services.
• Document all services in Electronic Health Record.
