Job summary

Northwestern Medicine is seeking a Staff Nurse to deliver age-specific, evidence-based nursing care from admission to discharge. You will provide direct patient care, collaborate with the health care team, and support patients and families in achieving realistic outcomes.

You will assess patient status, organize data for the plan of care, initiate discharge planning, educate patients, and document outcomes. The role requires ethical practice and participation in multidisciplinary meetings.

Qualifications

• BSN degree required.
• Current Illinois RN license.
• BLS certification.
• Ability to collaborate with others.
• Strong analytic abilities.

Responsibilities

• Assess and records patient status upon admission or point of care entry.
• Organizes the assessment health data so it is accurate, complete and accessible.
• Identifies problems that need immediate attention.
• Validates diagnoses with patient/family and team members.
• Documents diagnoses in a manner that facilitates the plan of care.
• Initiates and updates the Problem List at admission with age-specific care.
• Discusses the plan of care with patient/family.
• Begins discharge planning at admission and coordinates with team.
• Identifies patient teaching needs and arranges education.
• Maintains evidence-based, outcome-driven nursing interventions.
• Documents all assessments, interventions and patient responses.
