Job summary

UHS's Cedar Ridge in Bethany, OK seeks a Case Manager/Step-Down Navigator to meet with eligible adult patients and discuss the benefits of stepdown services as part of their continuum of care. The role coordinates between departments, supports discharge planning, and tracks eligibility data in Midas for reporting, while adhering to ANA Standards of Practice for Psychiatric Mental Health Care.

Qualifications

• High school diploma or equivalent required.
• Minimum one year of case management experience, preferably in the mental health field.
• Thorough understanding of the inpatient workflow and step-down process as it pertains to case management and discharge planning.
• Case Management Certification preferred.
• Valid Driver's License required.
• Must be computer proficient and have strong communications and analytical skills.
• Exposure to psychiatric patients who may exhibit violent/aggressive behavior.

Responsibilities

• Meet with eligible adult patients to discuss the benefits of stepdown services as part of their continuum of care.
• Coordinate between departments to ensure smooth step-down processes.
• Communicate eligibility to key leaders and associated departments.
• Routinely collect and enter eligibility data into Midas for trending reports.
• Assist therapists with discharge planning and develop referral sources.
• Perform duties in accordance with ANA Standards of Practice for Psychiatric Mental Health Care and facility policies.
