Job summary

Universal Health Services' Cedar Ridge Bethany is seeking a Case Manager/Step-Down Navigator to meet with eligible adult patients and discuss the benefits of stepdown services as part of their continuum of care.

The role coordinates between departments, communicates eligibility to senior leadership, assists with discharge planning, and supports referrals in alignment with psychiatric mental health standards.

Qualifications

• Bachelor’s degree in related field preferred.
• High school diploma or equivalent required.
• Minimum one year of case management experience required, preferably in the mental health field.
• Thorough understanding of the inpatient workflow process and step-down process as it pertains to case management and discharge planning preferred.
• Case Management Certification preferred.
• Valid Driver's License, required.
• Must be computer proficient and have strong communications and analytical skills.
• Exposure to psychiatric patients who may exhibit violent/aggressive behavior.

Responsibilities

• Meet with eligible adult patients to discuss the benefits of stepdown services as part of their continuum of care treatment.
• Coordinate between departments and communicate eligibility to senior leadership.
• Assist therapists with discharge process, develop referral sources, and perform duties in accordance with ANA Standards of Practice for Psychiatric Mental Health Care.
